The CDC publishes Vaccine Information Statements, referred to as VIS, for all vaccines. PVP wants to provide easy access to VIS for you prior to your child’s visits. A VIS can be accessed @ https://www.cdc.gov/vaccines/hcp/vis/current-vis.html. Direct links to individual vaccine VISs are pro- vided in the vaccine schedule below. A paper copy of a VIS can be provided to you at your child’s visit upon your request.

We recommend yearly physical exams after the age of 2. Please check with your insurance company regarding coverage. Vision, Hearing, Hemoglobin, Lipid and Developmental screenings are performed at age appropriate Well Child visits in accordance with American Academy of Pediatrics Recommendations.
