The CDC publishes a Vaccine Information Statement for every vaccine. They are referred to as a VIS.
VIS’s provide parents helpful information about each vaccine.
PVP doctors want to provide parents easy access to each VIS prior to their child’s visit.

Massachusetts requires that practices share their patient’s vaccine records with the MA State Vaccine Registry. If
you prefer to opt out of sharing your child’s vaccine records with other medical facilities, please ask a staff
